Why do you need my license number, state, and month/year of birth?

Physicians enrolling in the course are required to provider there license number and state as well as month and year of birth to receive AMA credit per the ACCME guidelines. Failure to provide this information can result in inability to claim CE credit.
How much does the course cost?

The course is $297, less than the cost of one 50 unit vial of Botox! The price is split over 3 months at $99 per month.
How long will I have access to the course?

Enrollment in this course includes access to all materials for a period of 12 months from the date of purchase. After this time, continued availability is not guaranteed.
